The Position: Home Health Physical Therapist

We are seeking a physical therapist responsible for improving a patient’s quality of life by planning, directing, and providing home care services. This licensed therapy professional will provide direct therapy care and manage a team of therapists and other professionals to treat patients/clients under a physician’s plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Nurse Practice Act, and regulatory entities. The therapist case manager’s goals are to rehabilitate and educate the patient to regain and retain independence in their home setting. The provided care plan will include all clinical care, symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.

Job Responsibilities:
  • Assess patient’s home environment for safety hazards and architectural barriers identify equipment needs.
  • Identifies functional problems and establishes goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
  • Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
  • Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance to agency policy.
  • Periodically re-evaluates and upgrades the patient’s program to maximize functional improvement.
  • Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
  • Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
  • Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
  • Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
  • Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
  • All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48) hours.
  • Will strive to maintain positive internal and external customer service relationships.
  • Must be able to demonstrate effective leadership skills.
  • Must demonstrate effective communication skills by conveying necessary information accurately, listening effectively and asking questions when clarification is needed.
  • Must be able to be depended upon to plan and organize work effectively and ensure its completion.
  • Must be able to demonstrate reliability by arriving to work on time and taking breaks in expected time frames.
  • Will be expected to meet all productivity requirements.
  • Must be able to demonstrate team behavior and must be willing to promote a team-oriented environment.
  • Is expected to represent the organization professionally at all times.
  • Must be able to demonstrate initiative, strives to continually improve processes and relationships.
Education and Experience:

Must be a graduate from an accredited Physical Therapy program and licensed by the state of Arizona.

Compensation: $75,000.00 - $125,000.00 per year
