The Nurse Practitioner is a member of a multidisciplinary team at Sunrise Health Services which is a leading home health agency in Phoenix, AZ with a reputation for quality care including a 4.5 out of 5.0 Medicare STAR quality rating.

Responsibilities:
- Conduct comprehensive patient assessments, including medical history, physical examination, and review of symptoms
- Order and interpret diagnostic tests, such as laboratory work and imaging studies
- Diagnose and treat acute and chronic illnesses, injuries, and conditions
- Develop and implement patient care plans in collaboration with the healthcare team
- Prescribe medications and other treatments as necessary
- Provide education and counseling to patients and their families on health promotion, disease prevention, and treatment options
- Perform procedures such as wound care, suturing, and administering injections
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records

Qualifications:
- Valid Nurse Practitioner license in the state of practice
- Master's degree in Nursing with specialization as a Nurse Practitioner
- Experience working as a Nurse Practitioner preferred
- Strong knowledge of physiology and pathophysiology
- Proficiency in conducting physical examinations and interpreting diagnostic tests
- Ability to develop and implement patient care plans based on assessment findings
- Excellent communication skills to effectively educate patients and collaborate with the healthcare team
- Familiarity with utilization review processes and procedures
- Experience in dermal fillers administration is a plus
- Knowledge of triage protocols for emergency situations
- Competency in specimen collection processing for laboratory testing
- Ability to perform procedures such as ventilating patients or performing spinal taps is desired but not required
